In the context of genetically susceptible backgrounds, a “danger signal” in the thymus, such as pathogen infection, may trigger the activation of TLR4-mediated signaling pathways, which in turn may lead to pathogenic and self-perpetuating inflammatory autoimmune response. Specifically, persistent activation of TLR4 in TECs and thymic DCs may promote an inflammatory state that contributes to increased expression, presentation and cross- presentation of AChR, leading to T-cell autosensitization and development of the autoimmune response. Close interactions between T-cells and APCs at the medullary HC areas could be critical for initiation of AChR-specific autoimmunity. Increased production of pro-inflammatory chemokines (e.g. CXCL13, CCL21, CCL17 and CCL22) may cause abnormal recruitment of DCs, T- and B-cells in the thymus, leading to hyperplastic changes and perpetuating the inflammatory state. <t>Th17-related</t> cytokines produced in the inflammatory microenvironment may impair Treg function, thus favoring perpetuation of the inflammatory process and contributing to the feedback loop that promotes maintenance of the autoimmune response in MG thymus.
